The wordart below, just right click and save.

Create a new canvas 700x700.

Resize 75% and keep it centered in your canvas.

Use your magic wand to select the inside of the frame.
Expand the selection with 5 pixels and invert the selection.
Paste paper 13 and hit delete.
Paste your close up tube and hit delete. (I used my fullsize tube resized to 75%)
Deselect and move your frame layer to the top.
Set the layermode of the tube to Overlay.

Copy paste element 78.
Leave it centered as top layer.

Repeat with 77 and 69.

Copy paste element 81 to the left side of your frame.

Copy paste element 42 to the left side of your frame.

Copy paste element 4 to the right side of your frame.
Copy paste element 63 over the previous flowers and move them down so the flowers cover the stems of the previous.
Erase the stems of this element.

Copy paste element 67.
Resize 75%, mirror and move to the bottom of your frame.

Copy paste element 61.
Leave it centered as your top layer.

Copy paste element 12 to the bottom of your frame.

Paste your tube on the left side of your frame. (I resized mine 30%)
Copy paste element 37.
Resize 65%, mirror and move to the right upperside of your frame.

Copy paste element 32 into your tag as background layer.
Leave it centered.

Add your wordart as top layer.

Crop your tag and give all elements a dropshadow of 1-1-40-5.

Add your name. Don't forget your copyright. And you're done!
